When you come for your appointment, you must bring the following:
- Medicare Card (Red/white/blue)
- List of all RX/OTC medications
- Email address
- Cell phone (must be able to receive a text)
- A list of all doctors/providers/hospitals
- Any questions or issues you need addressed
Without a cell phone, we won't be able to help you unless you have a medicare.gov account created before Oct. 1st.
